The C-NLOPB has issued a notice of non-compliance to ExxonMobil Canada and KCA Deutag Drilling Canada for a near miss on the Hebron platform one month ago.
On May 28, a hydraulic pin puller failed, resulting in a 15 pound puller rod being projected across a pipe deck, dropping down to the walkway below.
At the time, ExxonMobil said that the incident “had the potential for a fatality.”
The C-NLOPB says their review found that the puller rod was exposed to loads above the yield rating, and that the hydraulic rate capacity was also exceeded, resulting in the pin being fractured.
The board says neither company provided adequate information on how to execute the work safely.
Both are now required to perform an audit to determine the “failures and gaps in the management system” that contributed to the non-compliances, that they conduct a review of all jacks and similar tools on the platform to ensure equipment limitations are included in their procedures, and that they ensure all employees are trained in how to use the equipment.
As well, ExxonMobil must issue an incident bulletin to the local offshore industry.
The results of those actions must be submitted to the C-NLOPB by September 30.
