A lack of oversight and inadequate reporting was cause for concern for the Auditor General’s team while reviewing government-run long term care facilities.
The AG’s audit report released yesterday spanned a period from 2024 to 2026.
AG Denise Hanrahan told VOCM Open Line with Brian Callahan that the Patient Safety Act requires notice to the minister of adverse health events or occurrences that involve multiple patients or multiple regions.

“Under that requirement, nine reports were made to the department of that nature…It did make me question though, when I know that there were over 3,400 occurrences that resulted in either temporary or permanent harm to residents.”
“When you look at that quantity and only nine were considered adverse enough to tell a minister, it made my audit team want to look further into the details of these and to assess the process.”
